Researchers are advancing brain-computer interface (BCI) technology to restore communication for individuals with severe paralysis or speech impairments. 

Initial developments from the University of California, San Francisco (UCSF) involve an experimental implant capable of simultaneously decoding neural activity from the sensorimotor cortex to translate both speech and physical gestures. In studies published in Nature Neuroscience, this system used machine learning to allow a virtual avatar to replicate facial expressions and manual movements, such as nodding or waving, alongside spoken words. Tests with three patients showed significant potential, with one participant achieving 100% accuracy in decoding both speech and gestures during specific assessments.

Parallel efforts in the neurotechnology sector include Neuralink’s ‘VOICE’ clinical trial. Utilizing its N1 implant, Neuralink aims to convert brain signals into audible synthetic speech, targeting a conversation speed of approximately 140 words per minute. The U.S. Food and Drug Administration has designated Neuralink’s communication device as a ‘breakthrough device’ to expedite its development. Both the UCSF system and Neuralink’s technology remain in experimental or clinical trial stages and are not yet available for general medical use.
