Recent clinical trial findings presented at the American Society for Radiation Oncology (ASTRO) Annual Meeting highlight new approaches to treating brain metastases in lung cancer patients.

One phase III study involving 304 patients found that brain MRI surveillance alone resulted in a 40 percent improvement in cognitive failure-free survival compared to combining MRI with prophylactic cranial irradiation. Additionally, patients with small cell lung cancer (SCLC) receiving focused stereotactic radiosurgery (SRS) lived a median of 17 months, compared to nine months with advanced whole brain radiation therapy.

In a separate study from the Alliance A071801 trial, researchers found that fractionated SRS is more effective than single-fraction SRS for patients following brain metastasis resection. The fractionated group showed an 87 percent surgical bed control rate at one year and a median overall survival of 29 months, whereas the single-fraction group had 81 percent control and a median survival of 20 months. Experts suggest fractionation may become the standard of care, though single-fraction treatments remain an option for specific patient profiles.

## Timeline

### 2026-09-28: Fractionated radiation improves survival for brain metastasis patients

A phase 3 trial presented at the ASTRO Annual Meeting shows that fractionated stereotactic radiosurgery improves survival and surgical bed control compared to single-fraction SRS for brain metastases.

### 2026-09-28: SCLC brain metastasis treatments show improved survival and cognition in phase III trial

Phase III trial results show brain MRI surveillance alone improves cognitive failure-free survival in SCLC patients, while stereotactic radiosurgery may extend survival compared to whole brain radiation.
