The Brazilian Ministry of Health is implementing Ordinance GM/MS nº 11.685/2026 to modernize blood donation regulations, with a nationwide transition scheduled for October 2026. The updates aim to expand the donor pool and increase hospital supplies by adjusting eligibility criteria and reducing waiting periods.

Significant changes include removing the maximum age limit for healthy regular donors over 70. Waiting periods for various procedures have been reduced: tattoos, piercings, permanent makeup, and aesthetic procedures like Botox or fillers now require only a seven-day wait if performed in licensed facilities with valid sanitary permits; otherwise, the wait is four months. Piercings in the mouth or genital area require a four-month wait following removal. Other reductions include bariatric surgery (to six months), tuberculosis (to two years post-cure), and endoscopy or colonoscopy (to four months). The wait after sexual contact with casual or unknown partners will decrease from 12 to four months. Additionally, users of medications such as Ozempic, Wegovy, or Mounjaro may donate after a 14-day waiting period following a dose change or the cessation of adverse effects.

To enhance safety, the regulations mandate the ISBT 128 international standard and the NAT Plus malaria test developed by Bio-Manguinhos/Fiocruz, allowing individuals exposed to malaria-endemic areas to donate after 30 days. Cancer survivors (excluding melanoma and hematological neoplasms) may donate five years after treatment. Furthermore, the shelf life of platelet concentrates will be extended from five to seven days.
