Recent clinical trials are providing new insights into lifestyle management for patients with atrial fibrillation (AF).

The DECAF trial, conducted in Australia, Canada, and the United States, investigated the impact of caffeine on arrhythmia recurrence following electrical cardioversion. The study found that participants who consumed at least one cup of caffeinated coffee or an espresso shot daily had a 39 percent lower hazard of clinically detected recurrence over six months compared to those who abstained from caffeine. Researchers reported that recurrence occurred in 47% of the coffee-consuming group versus 64% of the abstinence group, suggesting that moderate caffeine intake may not increase adverse events and could be associated with lower recurrence rates.

In a separate study, the NEXAF trial focused on the benefits of physical activity. Results presented at the ESC Congress 2026 indicated that a one-year structured exercise program, involving both supervised high-intensity sessions and long-term home-based exercise with remote monitoring, significantly reduced the burden of atrial fibrillation and improved quality of life in symptomatic patients. Researchers noted that despite existing guidelines, there remains a significant implementation gap, as only approximately 1 in 10 eligible patients are currently referred for exercise-based rehabilitation.
