The U.S. Department of Health and Human Services (HHS) released a report titled ‘Wolves in White Coats: How Doctors and Hospitals Pushed and Profited from the Fraud of Gender Medicine.’ The report alleges that pediatric gender clinics and hospitals may have used misleading diagnostic codes, such as ‘precocious puberty’ or ‘unspecified endocrine disorders,’ to secure insurance reimbursement for medical transition procedures for minors, including puberty blockers, hormones, and surgeries.

The report estimates nearly $120 million in submitted charges involving minors since 2019 and describes a ‘captive patient’ dynamic that creates long-term revenue streams for providers. It also criticizes previous federal policies that promoted access to these procedures.

In response to these findings and citing safety concerns and evidence gaps, HHS announced a final rule ending the use of federal Medicaid and CHIP funds for medical transition procedures for patients under 19, effective October 13. Patients currently on hormone therapy will be permitted a six-month taper period.
