Research presented at the United European Gastroenterology Week indicates that specific antibody patterns in the blood may predict the onset of inflammatory bowel disease (IBD), such as Crohn’s disease and ulcerative colitis, up to ten years before clinical diagnosis.

Studies involving U.S. military personnel, as well as cohorts in Sweden and Norway, suggest that the immune system undergoes pathological changes long before symptoms or intestinal damage occur. Specifically, individuals who later developed Crohn’s disease showed elevated antibody responses against flagellin and herpesviruses, such as the Epstein-Barr virus. Those developing ulcerative colitis exhibited distinct profiles, including reactions to specific proteins within the body. Additionally, gut microbiome composition has been identified as a potential classifier for predicting aggressive disease courses in newly diagnosed ulcerative colitis patients.

Further analysis of 2,000 blood samples revealed that future IBD patients exhibited significantly lower antibody diversity compared to healthy controls. A computer model utilizing these findings achieved accuracy scores of 0.90 for Crohn’s disease and 0.84 for ulcerative colitis.

Building on these findings, research presented at UEG Week 2026 involving 500 individuals reaffirmed that reduced antibody diversity can appear a decade before diagnosis. Complementary research published in the New England Journal of Medicine by teams from the University of Oxford, Newcastle University, and Cambridge University Hospitals NHS Foundation Trust identified specific biological mechanisms in a study of over 4,900 people. This study found that some patients produce antibodies that interfere with interleukin-10 (IL-10), a protein essential for regulating inflammation. These discoveries suggest that IBD may consist of several biologically distinct conditions, potentially allowing for personalized treatments tailored to a patient's specific biological cause.
