Italy is implementing a reorganization of local healthcare services through the establishment of community health hubs, known as ‘Casa di Comunità’. Under the framework of PNRR Mission 6 and DM 77 of 2022, these hubs aim to standardize territorial assistance and create a cohesive approach to proximity care.

In the Marche and Emilia-Romagna regions, services are being centralized to improve access. In Jesi, a new hub opened on September 23 in via Aldo Moro, opposite the Carlo Urbani Hospital. This facility integrates medical continuity, home care, and a single point of access, hosting specialist clinics for cardiology, physiotherapy, ophthalmology, orthopedics, and urology, though blood collection remains at the via Guerri site. In Imola, the local model is shifting from an Urgent Care Center to an AFT clinic starting October 1. Health director Paolo Tarlazzi noted this new model will feature two doctors providing 12-hour daily coverage, seven days a week.

Expansion continues through urban regeneration and national funding. In Prato, Tuscany, the Asl Toscana centro commission has selected a site in the Fabbricone area for a new hub supported by European Fesr 2021-2027 funds. In the Valdaso area of Fermo, a renovated 1,650-square-meter center was inaugurated with a 1.6 million euro investment, largely funded by the PNRR.

New initiatives are also targeting vulnerable populations. In Bologna, a network integrating street nurses with solidarity dental laboratories is emerging to reach migrants and those facing economic hardship. Meanwhile, in Montalto Uffugo, discussions regarding the ‘Case della Comunità’ model emphasize a global approach that integrates medical services with social support, prioritizing the home as a primary place of care.
