Medical professionals are increasingly advocating for the use of Janus kinase (JAK) inhibitors as a primary treatment for skin conditions such as vitiligo, atopic dermatitis (AD), and alopecia areata. 

In the treatment of vitiligo, clinicians distinguish between topical and oral applications. Topical ruxolitinib cream is preferred for localized disease affecting less than 10% of the body surface area, while oral agents like upadacitinib, ritlecitinib, and povorcitinib are recommended for more progressive or widespread cases. While oral therapies require caution in patients with high cardiovascular risk or high BMI, phase 3 trials have shown few systemic adverse effects.

Experts at the Fall Clinical Dermatology Conference are further suggesting that JAK inhibitors may be preferable to systemic steroids. They note that even short courses of systemic steroids are linked to risks such as sepsis, fractures, and blood clots. A retrospective study of 376 patients treated with various JAK inhibitors for AD and alopecia areata found no major cardiovascular or malignancy signals, suggesting a reassuring safety profile in real-world clinical practice.
