Initial guidance from experts at the University of Pittsburgh School of Medicine and Ohio State University Wexner Medical Center identified five critical symptoms: sudden and persistent pain, chest pain, leg pain accompanied by swelling or redness, a tearing sensation in the upper back, and specific lower abdominal pain. These symptoms may indicate serious conditions such as heart attacks, pulmonary embolisms, deep vein thrombosis (DVT), aortic issues, or appendicitis.

Subsequent warnings reinforced these concerns, with Dr. Michael Turturro noting that intense pain accompanied by shortness of breath, nausea, or dizziness could indicate severe cardiovascular issues. Additionally, the Slovak Society of General Practitioners highlighted that calf pain involving warmth, redness, or swelling may signal DVT.

Expanding on these warnings, emergency medicine specialists have noted that pain intensity alone is not a reliable indicator of severity, as even mild pain can signal serious underlying conditions. 

In addition to pain symptomology, pharmacists have raised concerns regarding medication safety. Dr. Christine Cadiz warned that taking non-steroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, on an empty stomach can significantly increase the risk of stomach issues, including heartburn, indigestion, and ulcers. This risk occurs because ibuprofen blocks the COX-1 enzyme, which produces prostaglandins that protect the stomach lining. To mitigate these risks, experts recommend taking these medications with food, milk, or water to provide a buffer for the digestive tract.
