On October 23, 2026, a 45-year-old machine operator, José Ronaldo Silva de Carvalho, died following an accident at the Pilar underground mine in the Caraíba unit in Jaguarari, Brazil. Preliminary reports indicate the operator may have been struck by a motor grader after stopping it on a ramp to perform an activity.

In response to the incident, ERO Caraíba suspended all production activities at the Pilar, Vermelhos, and Surubim mines, as well as the plant, for 24 hours to review workplace safety.

Following the fatality, the Labor Public Ministry (MPT) in Bahia opened an inquiry to investigate the circumstances of the death.
