Clinical research in the United Kingdom is challenging established treatment protocols for acute bacterial endophthalmitis. A phase 2 feasibility trial, known as the EVIAN trial, conducted by researchers from University College London and Moorfields Eye Hospital, evaluated the effectiveness of early vitrectomy compared to the conventional regimen of repeated intraocular antibiotic injections.

The study, which involved 63 patients across 21 UK hospitals, found that patients receiving early vitrectomy within 48–96 hours of diagnosis showed significantly better outcomes. At six months, those in the surgical group achieved a median visual acuity gain of 40 letters, more than three times the median gain of 13 letters observed in the antibiotic-first group.

Further research has indicated that topical antibiotics may not reduce the incidence of endophthalmitis following intravitreal injections, suggesting that prevention should instead prioritize rigorous antiseptic techniques using povidone-iodine.
