In early November 2026, regional deputy Cristina Ciminnisi warned of a collapsing public healthcare system in the province of Trapani. She reported that emergency rooms were operating with only 30 doctors despite a requirement for 80, creating a deficit of approximately 50 medical professionals. Ciminnisi also cited a backlog of nearly 24,000 pending procedures and criticized the local health authority and regional Health Assessor Caruso for failing to address the crisis.

In response to staffing shortages, the Sicilian regional government approved a plan to hire 2,917 permanent healthcare professionals. Signed by Health Assessor Marcello Caruso, the initiative is supported by 160 million euros from the National Recovery and Resilience Plan (PNRR). The recruitment drive focuses heavily on nurses, with 1,571 positions available, alongside 205 doctors, 172 socio-sanitary operators, 146 social workers, and 780 technical and administrative staff. The funding will be distributed across nine provincial health authorities to support community houses and hospitals, with the largest allocations directed toward Palermo, Catania, and Messina.

Alongside this recruitment, Regional President Renato Schifani has issued a directive to tighten healthcare spending to address a projected deficit. Schifani warned that general managers of healthcare companies who fail to meet economic balance objectives may face dismissal procedures. While current estimates suggest a potential deficit of 650 million euros by 2026, the administration aims to use containment measures to reduce this to approximately 400 million euros.

As the plan to redesign territorial healthcare through new community houses and hospitals progresses, the Order of Biologists of Sicily has raised concerns regarding the staffing requirements. The Order argues that biologists are being excluded from the recruitment calls and is calling on regional institutions to include the profession, noting that their expertise in genetics and prevention is essential for local public health.
