Stanford researchers have published findings suggesting a link between estrogen-only hormone therapy and a reduced risk of dementia and Alzheimer’s disease in women. An observational study involving over 21,000 women analyzed medical records and autopsy data to compare estrogen users against non-users. The research indicates that women receiving estrogen therapy had a 39 percent lower chance of a clinical dementia diagnosis and a 33 percent lower risk of memory and functional decline. Biological markers examined through brain autopsies showed that estrogen users exhibited 35 percent fewer Alzheimer’s-related changes, such as amyloid plaque accumulation.

Recent complementary research from the University of Delaware has added mechanical context to these findings, demonstrating via magnetic resonance elastography (MRE) that estrogen fluctuations influence the brain by reshaping connections between neurons, particularly in the hippocampus.

New large-scale research from the University of East Anglia and the University of Exeter has expanded on these observations. Analyzing data from over 180,000 women in the UK Biobank, researchers found that hormone replacement therapy (HRT) users had a 16 percent lower risk of developing Alzheimer’s compared to non-users. The most significant reduction, at 26 percent, was observed in women who had undergone surgical menopause. 

While these findings are promising, experts emphasize that the results demonstrate a correlation rather than direct causation. Medical professionals advise that these findings should not immediately alter current hormone therapy practices, as further studies are required to determine how these changes directly influence cognitive functions and to establish definitive preventative roles.
