Javier Medrano, a former manager of Tesla’s Full Self-Driving (FSD) test fleet in Houston, has filed a federal lawsuit alleging systemic safety failures and whistleblower retaliation. Medrano claims that Tesla expanded its autonomous vehicle program without sufficient oversight, resulting in a “dangerous 38:1 operator-to-manager ratio” that violated internal safety guidelines. He asserts that this understaffing caused extreme driver fatigue and contributed to a March 2025 incident where a test vehicle struck a member of the public.

Medrano, who was terminated in May 2025 after raising these concerns, alleges that Tesla retaliated against him by firing him and stripping a pending stock award. In addition to the private litigation, the National Highway Traffic Safety Administration (NHTSA) has expanded its investigation into Tesla’s FSD system. The regulatory agency is examining the technology’s ability to detect and warn drivers during conditions of limited visibility, such as fog, glare, and dust, following nine reported accidents.
