The Trump administration announced that approximately 20.8 million Original Medicare Part B enrollees in the United States will receive a one-time $90 payment in October 2026. Officially termed the ‘Medicare Improvement Fund Premium Rebate’, the initiative is funded by the Medicare Improvement Fund, a $2 billion pool established by Congress in 2008.

Most eligible recipients are scheduled to receive the funds via direct deposit around October 8, while those without direct deposit will receive a paper check from the U.S. Treasury later in the month. Eligibility is limited to Original Medicare Part B beneficiaries living in the U.S. who do not receive Medicaid assistance for their premiums or pay the Income-Related Monthly Adjustment Amount (IRMAA). Medicare Advantage members are excluded from this payment, which is not a permanent reduction in premiums or a recurring benefit.
