Zambia and the United States have signed a five-year health cooperation Memorandum of Understanding (MOU) to strengthen Zambia’s healthcare system. The agreement follows months of negotiations regarding national sovereignty, specifically concerning the sharing of biological samples and individual patient data. Under the final terms, Zambia is not required to share personally identifiable information or biological specimens with Washington, limiting information sharing to what is necessary for monitoring and evaluating U.S.-funded programmes.

The financial scale of the partnership involves combined commitments estimated between US$2.5 billion and US$3.6 billion. The United States is expected to contribute approximately US$1.5 billion over five years to support initiatives such as HIV/AIDS and malaria programmes, maternal and child health services, and disease outbreak preparedness. To ensure sustainability, Zambia is expected to increase its domestic health expenditure by roughly US$975 million, reaching approximately US$2.1 billion over the period. This domestic increase aligns with a broader U.S. foreign aid policy shift toward transitioning recipient nations to self-sufficient financing models.

The MOU aims to facilitate the recruitment of 40,000 health workers, comprising 15,000 frontline workers and 25,000 community health workers. Additionally, the agreement seeks to support Zambia in managing its own procurement of essential medicines and expanding digital systems for health threat detection. Zambian officials have reiterated that this health pact remains deliberately decoupled from ongoing negotiations regarding the country’s critical mineral resources.

Recent implementation efforts include U.S.-funded shipments of Lenacapavir, a long-acting HIV prevention drug, to improve treatment adherence. Concurrently, private sector initiatives such as donations to the non-profit Kupirira aim to establish sustainable procurement models for antiretroviral drugs to protect the estimated 1.2 million people living with HIV in Zambia.
