American College of Surgeons Issues New Standards for Level IV Trauma Centers

The American College of Surgeons Committee on Trauma (ACS COT) has published a new guideline, *Resources for Optimal Care of the Injured Patient: Level IV Trauma Center Standards (2026)*. The 30 standards, organized into seven domains—including administrative commitment, program governance, staffing and equipment, patient‑care protocols, data surveillance, performance improvement, and professional education—are intended to help Level IV trauma centers evaluate capabilities, close gaps, and improve care delivery.

Level IV centers often serve as the first point of contact for injury patients in rural and remote areas, providing early stabilization, emergency interventions, and coordination of transfers to higher‑level facilities. The standards address continuous emergency‑department physician coverage, airway management, pediatric readiness, access to blood products, transfer protocols, trauma‑registry operations, and ongoing clinician education. Although the ACS does not certify Level IV centers, the guidance is meant to complement state trauma‑system requirements, with states retaining authority over designation and oversight.
