Augustinópolis Regional Hospital faces lawsuit after fatal transfusion error
The Public Prosecutor's Office of Tocantins (MPTO) has filed a civil public action against the State following the death of a 78-year-old patient at the Augustinópolis Regional Hospital. The patient reportedly received a blood transfusion incompatible with her blood type; records indicated she was O+, but an A+ bag was administered. Approximately 50 milliliters were given before the error was identified.
An investigation by the Regional Nursing Council highlighted several systemic failures at the facility, including the absence of a transfusion committee, outdated written procedures, a lack of ongoing staff training for nearly two years, and incomplete records. The MPTO is seeking urgent measures to correct these flaws and is requesting R$ 1 million in collective moral damages to be allocated to state health or diffuse rights funds. The Civil Police are also investigating the incident to determine individual responsibilities.
