Biologic therapies for Hidradenitis Suppurativa undergo comparative analysis

Clinical discussions and recent analyses are evaluating the optimization and comparative effectiveness of biologic therapies for Hidradenitis Suppurativa (HS). Currently, three agents—adalimumab, secukinumab, and bimekizumab—possess high-confidence evidence for achieving clinical response (HiSCR 75) compared to placebos.

A matching-adjusted indirect comparison published in the Journal of the European Academy of Dermatology and Venereology examined the performance of these drugs. The analysis indicated that bimekizumab showed higher odds of clinical response at various thresholds, greater quality-of-life improvements, and more significant reductions in inflammatory nodules and draining tunnels when compared to both secukinumab and adalimumab. However, researchers noted that these findings are not based on direct head-to-head trials.

Clinical management also faces challenges regarding treatment adherence. Data indicates that approximately 60% of patients discontinue adalimumab within one year of starting therapy, with younger age, female sex, and Medicaid coverage identified as potential risk factors. Clinicians are encouraged to focus on patient counseling to maintain therapy in those responding well and to consider the role of surgery and layered treatments in comprehensive HS management.
