Brain white matter health may slow cognitive decline and buffer gray matter loss

Recent research suggests that the health of brain white matter may play a critical role in mitigating cognitive decline and buffering the effects of gray matter loss in older adults.

A secondary analysis of the US-based POINTER randomized clinical trial, published in JAMA Network Open, found that a structured multidomain lifestyle intervention could slow age-related changes in brain white matter. The intervention, which included supervised exercise, dietary counseling, cognitive and social engagement, and health coaching, significantly reduced the increase in white matter free water over time in participants younger than 70 years compared to a self-guided educational group.

Complementary research involving a community-based population in India, published in Alzheimer’s and Dementia, indicates that healthier superficial white matter is consistently associated with stronger language skills. This study suggests that the integrity of white matter wiring may help soften the cognitive impacts caused by gray matter reduction. Researchers utilized advanced diffusion MRI to assess neurite density and free water to understand these structural brain resilience dynamics.
