Bulgarian Ministry of Health proposes major healthcare reorganization

The Bulgarian Ministry of Health, led by Minister Katya Ivkova, has proposed a large-scale reorganization of the national healthcare system. The draft amendments to the Health Act aim to optimize administrative structures, clarify responsibilities, and improve institutional coordination. Key proposals include transforming Regional Health Inspectorates into territorial directorates, integrating the National Center for Public Health and Analyses, and merging two national health centers. The plan also involves consolidating medical expertise files and creating a unified electronic register for healthcare facilities.

The reorganization includes a reduction of 440 staff positions, including 200 in emergency services. The project is currently open for public consultation for a 14-day period, ending September 17, 2026, with the goal of implementation by January 1, 2027.

The proposal has faced significant opposition from the Democratic Bulgaria parliamentary group. Represented by Katya Paneva, the coalition announced it would not support the reform in its current form, citing risks of excessive centralization of power in the hands of the Minister. Critics also expressed concerns regarding potential administrative chaos in the regions, lack of transparency in drug pricing and reimbursement processes, and the potential for weakened health oversight.
