California Medi-Cal to implement new work requirements and eligibility reviews in 2027

California is preparing for significant changes to its Medi-Cal program beginning in 2027, driven by new federal requirements. Under the updated implementation timeline, certain members aged 19 to 64 without children or disabilities will be required to meet work or community engagement standards to maintain coverage. Qualifying activities include earning at least $580 per month or completing 80 hours of monthly activities such as volunteering, job training, or attending school. Exemptions apply to pregnant and postpartum individuals, parents of young children, people with disabilities, and certain other groups.

Additionally, starting March 1, 2027, some adults will face eligibility reviews twice a year rather than annually. Some members, including certain immigrants, will also transition from managed care plans to fee-for-service models on January 1, 2027.

In response to these shifts, state lawmakers are considering a proposal to increase transparency by identifying large companies with at least 100 employees enrolled in Medi-Cal. Supporters of the measure argue it would highlight businesses whose staff rely on taxpayer-funded healthcare, while opponents contend that eligibility is determined by household income rather than employer status.
