China faces challenges in social security and long-term care insurance expansion

China is navigating significant structural challenges as it expands its social security and long-term care insurance (LTCI) systems. Experts note that while LTCI is moving toward nationwide implementation following a decade of pilot programs, the system faces six major pain points: heavy reliance on medical insurance funds, significant disparities in funding ratios between cities, inconsistent coverage levels for different disability severities, poor reach in rural and mountainous areas, varied service offerings, and risks of regressive fiscal transfers.

Simultaneously, new regulations regarding social security enforcement are taking effect. The ‘National Tax Administrative Penalty Discretionary Standards (2026 Edition)’ will implement unified enforcement, allowing for fines of up to three times the amount of unpaid social security contributions. This move aims to standardize penalties and prevent inconsistent enforcement across different regions.

Concerns have also been raised regarding the sustainability of the pension system. With an aging population expected to add approximately 382 million people aged 60 or older between 2022 and 2036, the ratio of contributors to beneficiaries is shifting. Additionally, discussions have emerged regarding the accuracy of wage statistics used to determine contribution bases, as well as reports of local governments misappropriating social security funds.
