Developers aim to utilize psychological discomfort as a core emotional component of the experience, focusing on themes of violence, sexuality, and the relationship between pain and pleasure. Narrative Director Antony de Fault stated that the team seeks to “put people at ease” through uncomfortable situations within the safe space of a video game, while emphasizing a distinction between unhealthy cult dynamics and consensual BDSM practices.

Gameplay features brutal combat involving melee weapons, firearms, and the “Genesis Configuration,” which allows players to use the supernatural powers of the puzzle box to manipulate the environment and enemies. Players must manage resources, dodge attacks, and use abilities like the “Hell Chains” to survive encounters with various terrors.
