CMS models drive growth in value-based home health care

Federal models from the Centers for Medicare & Medicaid Services (CMS) are increasingly driving the growth of value-based care in the home health sector. Experts note that CMS-led initiatives, such as the home health value-based purchasing model and the Transforming Episode Accountability Model, are providing clearer paths for risk-based partnerships than Medicare Advantage or managed Medicaid plans.

While many older adults express a preference for aging in their homes rather than in nursing facilities, significant barriers to accessing home and community-based services (HCBS) remain. Financial constraints, lack of paid support, and systemic inequities affect access, particularly for marginalized communities and individuals with limited income. As functional limitations increase with age, the demand for effective HCBS and predictable federal care models continues to grow.
