CMS releases 2025 MIPS scores and proposes MIPS transition

The Centers for Medicare & Medicaid Services (CMS) has released the 2025 Merit-based Incentive Payment System (MIPS) final scores and performance feedback. These scores will determine the payment adjustments for eligible clinicians and group practices in 2027. Providers who believe their scores or adjustments were calculated incorrectly have 60 days to file a targeted review with CMS.

In a separate regulatory development, CMS has proposed sunsetting traditional MIPS reporting after the 2028 performance period. Under this proposal, MIPS Value Pathways (MVPs) would become the sole reporting option for clinicians outside of an alternative payment model starting in the 2029 performance period.

Practices intending to report via an MVP for the 2026 performance year must register by November 30. The American Medical Association has urged physicians to participate in a CMS survey regarding MVP adoption, particularly if they oppose the retirement of traditional MIPS. While the timeline for transitioning to MVPs has been signaled since 2019, this represents the first formal rule to establish a specific sunset date.
