Colombia health system faces projected $3.7 trillion peso funding shortfall

The financial sustainability of the Colombian health system is under scrutiny following warnings from the National Association of Businessmen of Colombia (Andi). Andi President Bruce Mac Master stated that the adjustment decreed for the 2026 Capitation Payment Unit (UPC) will be insufficient, resulting in a projected shortfall of $3.7 trillion pesos.

According to Andi's analysis, while $72.4 trillion pesos were contemplated for insurance, only $49.9 trillion pesos come from direct national contributions. This leaves a margin of only 5% to increase the UPC for the coming year, significantly lower than the 15.6% increase requested by sector actors.

Additional warnings have been issued by other entities. Anif previously estimated that the 2025 UPC required a 16.4% increase, far exceeding the decreed 5.36%, and calculated an accumulated deficit exceeding $19.8 trillion pesos over the previous three years. In response to these pressures, the Ministry of Health has requested an increase of $13.9 trillion pesos for the 2027 budget to cover operational and investment needs.
