Basque Country colon cancer screening cuts mortality by half
A biennial fecal immunochemical test (FIT) screening programme for colorectal cancer was introduced in the Basque Country in 2009, targeting residents aged 50 to 69. Participation reached about 70%, well above the national average.
A population‑based study covering 2004‑2024 examined roughly 2.2 million people and 438 000 deaths, of which 16 298 were due to colorectal cancer. The analysis found that mortality among the screened age group fell from 39.7 to 19.8 deaths per 100 000, a 50.1 % reduction, and overall mortality dropped from 32.8 to 23.1 per 100 000. More than 70 % of cancers detected were in stages I or II, and the programme is estimated to have prevented over 6 000 deaths. The mortality decline became apparent three years after the programme’s start, around 2012.
