FDA warning letters to drug firms rise 59% in 2025

The U.S. Food and Drug Administration (FDA) issued 303 warning letters to drug and biologics firms in 2025, marking a 59% increase from the previous year. An analysis of public FDA postings from 2021 to 2026 indicates that documentation, recordkeeping, and data integrity issues are pervasive in these enforcement actions.

In every year analyzed, at least 94% of the classified warning letters cited some form of documentation or data integrity problem. While these issues do not automatically imply that a medication is unsafe, unreliable records create gaps in the evidence required to prove that medicines were properly manufactured and tested according to quality standards.
