Finerenone Label Expansion Broadens CKD Treatment and Elevates Pharmacist Role

The FDA is considering a label expansion for finerenone, a non‑steroidal mineralocorticoid receptor antagonist, to include patients with chronic kidney disease (CKD) who do not have type 2 diabetes. This change would substantially increase the pool of eligible patients and allow clinicians to address kidney damage caused by hypertension and other primary kidney diseases.

Pharmacists are taking on expanded clinical responsibilities as the label widens. They are leading medication‑therapy management, closely monitoring serum potassium and estimated glomerular filtration rate to prevent hyperkalemia, adjusting dosages, and identifying patients with persistent albuminuria. By working alongside nephrologists and primary‑care providers, pharmacists aim to improve access to specialty care, mitigate cardio‑renal risks, and delay the need for dialysis.

The combined effort is expected to enhance patient safety, increase adherence to therapy, and broaden preventive kidney care across health‑systems.
