Germany: Significant quality gaps found in shoulder replacement surgeries
A study by the Scientific Institute of the AOK (WiDO) reveals substantial quality differences among German hospitals performing shoulder joint implantations. Analyzing data from 12,844 AOK-insured patients across 178 clinical sites between 2020 and 2024, the evaluation found an overall complication rate of 8.6 percent.
Complications, such as infected prostheses, blood vessel injuries, bone fractures, or joint dislocations, occurred in 7.1 percent of cases. The study highlights a stark disparity in outcomes: the top-performing fifth of clinics reported a complication rate of only 3.6 percent, whereas the bottom fifth saw rates as high as 14.5 percent—a fourfold difference.
Unplanned follow-up surgeries were required for 4.5 percent of patients within one year. To assist with patient decision-making, the AOK has integrated these results into its health navigator, allowing patients to view the quality of various surgical procedures at specific hospitals. Experts emphasize that high case numbers and clinical experience are critical factors for successful outcomes in endoprosthetic procedures.
