In Germany, patients taking at least three systemic medications are entitled to an individual, standardized medication plan to help manage complex treatments. While these plans are typically initiated by general practitioners, they are often incomplete or outdated, frequently omitting medications prescribed by other doctors.

To address this, pharmacies can review these plans to ensure accuracy, which helps prevent drug interactions, duplicate prescriptions, and prescribing cascades. For patients taking five or more regular medications, pharmacies offer expanded medication counseling. This service is provided free of charge to patients as it is covered by health insurance providers.
