Germany to transition healthcare telematics to cloud-based TI-Gateway by 2030

Germany's healthcare telematics infrastructure is undergoing a major restructuring. The decentralized Einbox-Konnektor is set to be replaced by a centralized cloud service known as the TI-Gateway. While originally targeted for 2025, this transition has been rescheduled for completion by the end of 2030. The migration is intended to occur alongside regular operations to prevent service interruptions and provide a more stable technical foundation for applications such as the electronic patient record (ePA) and e-prescriptions.

However, the transition presents financial challenges for medical facilities. Dr. Peter Gocke of Charité noted that annual operating costs for a large hospital could reach six-figure Euro amounts, a burden that may particularly impact small and medium-sized clinics. Additionally, a Gematik study indicated mixed results regarding current infrastructure stability, with only 57 percent of clinics using a TI-Gateway reporting a noticeable improvement in stability, noting an average of 25 disruptions per month in 2025.
