Medicare Launches $50 GLP‑1 Weight‑Loss Drug Pilot for Seniors

The U.S. Centers for Medicare & Medicaid Services (CMS) began a temporary pilot called the Medicare GLP‑1 Bridge on July 1 2026. The program runs through Dec 31 2027 and offers a $50 monthly copayment for selected anti‑obesity medications – Wegovy (injectable and tablet), Zepbound KwikPen, and Foundayo – to eligible Medicare Part D or Medicare Advantage beneficiaries.

Eligibility requires a body‑mass‑index (BMI) of 35 or higher, or a BMI of 30‑34.9 with certain high‑risk conditions (heart failure, uncontrolled hypertension, chronic kidney disease), or a BMI of 27‑29.9 with other comorbidities such as pre‑diabetes, prior heart attack, stroke, or peripheral artery disease. Applicants must obtain prior authorization and be enrolled in a medically supervised weight‑management program.

CMS estimates about 3.8 million beneficiaries meet the criteria, representing up to billions of dollars in drug costs (projected $1.3‑10 billion depending on uptake). The copayment does not count toward the Medicare deductible or out‑of‑pocket maximum, and the program excludes patients already covered for GLP‑1 drugs for diabetes, sleep apnea, or fatty liver disease. Experts note the need for monitoring older adults for muscle loss, bone density, and overall safety, while CMS plans to track participation and outcomes to inform possible future coverage extensions.
