GPIAAF issues safety recommendations after fatal Lisbon elevator accident

The Aircraft and Railway Accident Prevention and Investigation Office (GPIAAF) has formalized six safety recommendations following the Glória elevator accident in Lisbon, which occurred on September 3, 2025. The accident resulted in 16 deaths and approximately 20 injuries involving both national and foreign citizens.

The recommendations include four directives for Carris and two for the Institute of Mobility and Transport (IMT). Carris has been advised to review its internal control systems regarding the acquisition and application of critical safety components, as well as to conduct a critical review of its safety management system for electric modes, including elevators and funiculars.

In response, Carris announced it has launched an external audit of electric mode maintenance, a project to optimize its Integrated Management System, and a material categorization project. The company is also implementing organizational and procedural measures to strengthen safety management for its electric transport services, including the Santa Justa elevator.
