Gwanak-gu implements AI and pharmacist services for elderly medication safety

Gwanak-gu is enhancing medication safety for elderly residents and integrated care recipients through its ‘Smart Visiting Pharmacy Service.’ This initiative combines artificial intelligence (AI) technology with the expertise of local pharmacists to manage polypharmacy risks, specifically targeting individuals taking five or more types of medication.

The program utilizes an AI-based medication management application called ‘PhilTalk’ to generate medication safety reports. Based on these reports, high-risk individuals receive personalized care, including home visits by a team consisting of a pharmacist and a public health nurse. These teams check for duplicate prescriptions, inappropriate usage, and provide consultation on proper medication methods.

To support this, Gwanak-gu established a public-private-industrial cooperation system through agreements with the Gwanak-gu Pharmaceutical Association, the non-profit Neulpeum Gachi, and the ICT healthcare company Pimatch. So far this year, the district has issued 170 AI medication safety reports and provided one-on-one home visiting services to 75 people. The district also operates mobile consultation centers at senior centers and welfare facilities to provide education and identify residents in need of integrated care.
