Healthcare providers work to prevent medication errors in elderly care
The Health and Youth Care Inspectorate (IGJ) has warned that medication errors among vulnerable elderly people living at home are preventable. These errors often occur due to discrepancies in medication data between general practitioners, pharmacies, and home care services, particularly during transitions such as hospital discharges or changes in medical staff.
To mitigate these risks, the IGJ recommends increased investment in cooperation and clearer agreements regarding the responsibilities of different healthcare providers. A key issue is the lack of interoperability between various digital systems, which can lead to providers working with outdated or incorrect information. Additionally, the inclusion of self-care medications in official administration lists is necessary for home care to provide adequate support.
In the Helmond and De Peel regions, organized care programs are already in place to address these challenges. These initiatives involve multidisciplinary collaboration between general practitioners and organizations such as De Zorgboog and Savant Zorg. These programs focus on sharing medication data digitally and conducting joint discussions regarding polypharmacy to ensure safer medication use for vulnerable seniors.
