Herlev and Gentofte Hospital nutrition project reduces readmissions

A nutrition-focused initiative at Herlev and Gentofte Hospital, titled ‘Kom mæt hjem 2.0’, has demonstrated success in reducing hospital readmissions among elderly, vulnerable patients. Preliminary results from January 2025 to May 2026 indicate a 15 percent reduction in the risk of patients being readmitted within 30 days of discharge.

The project, which received 4.6 million DKK in funding from Region Hovedstaden, involves collaboration with Herlev Municipality and eight surrounding municipalities. Beyond improving patient outcomes, the initiative has shown economic benefits; after accounting for project expenses, it has generated a net saving of approximately 100,000 DKK. Officials suggest the program could become self-financing through the reduction in readmission costs alone.
