Hiperdia Program strengthens chronic disease care in Campo Grande
The Hiperdia Program is strengthening the continuous monitoring of patients with hypertension and diabetes within the health units of Campo Grande. By providing close follow-up, the program aims to identify health changes, guide lifestyle adjustments, and prevent complications before they escalate.
At the USF Aero Itália unit, weekly meetings include blood pressure monitoring, exam evaluations, prescription renewals, and guidance on chronic disease management. Municipal Health Secretary Marcelo Vilela emphasized that this close connection with the Unified Health System (SUS) allows teams to understand patients personally, which is vital for managing chronic conditions.
According to e-SUS data from the Collective Activities Report, the municipal health network recorded 7,941 group service sessions between January and August of this year, involving 97,212 participants in collective activities.
