Hospital-to-Home program expands to three Windsor‑Essex hospitals

The Hospital-to-Home (H2H) partnership, launched at Windsor Regional Hospital in October 2025, is being extended to Erie Shores HealthCare and Hôtel‑Dieu Grace Healthcare, covering all three major hospitals in the Windsor‑Essex region of Ontario.

Delivered in partnership with Saint Elizabeth Health, H2H offers patients up to 16 weeks of post‑discharge guidance so they can recuperate safely at home. "Partnerships like H2H demonstrate the value of regional collaboration in strengthening care close to home," said Katelyn Dryden, interim President and CEO of Erie Shores HealthCare. "Having a coordinated approach to care for our patients is a vital piece in ensuring positive patient and family outcomes," added Bill Marra, President of Hôtel‑Dieu Grace Healthcare.

The provincial government announced an almost $2 million investment to fund the expansion, part of a $1.1 billion commitment to boost home health‑care services across Ontario. Since its launch, 303 patients have participated in the program.
