Japan social security: Medical insurance premiums and nursing care revisions

In Japan, medical insurance premiums for the elderly are projected to rise, with the national average for the 2026 and 2027 fiscal years expected to reach 7,989 yen per month. To assist those facing financial hardship due to disasters, unemployment, or significant income loss, systems for premium reductions and collection deferrals are available. Some municipalities may also reduce income-based portions if income drops by 30 percent or more compared to the previous year.

Simultaneously, discussions are underway regarding the 2027 fiscal year nursing care compensation revisions. The Ministry of Health, Labour and Welfare has proposed a schedule to finalize basic principles by December 2026, with formal reports expected around January 2027. Key focus areas for the upcoming revisions include addressing rising prices and wages, adapting to population decline, enhancing community-based care, improving workplace productivity, and ensuring the long-term sustainability of the system.
