Klebsiella pneumoniae brain abscess reveals heterovirulence evolution
A 63‑year‑old Romanian woman suddenly lost vision in her right eye and was hospitalized. Imaging identified an ocular infection and a brain abscess in the parietal lobe. Laboratory tests from urine, blood and the abscess yielded the same bacterium, Klebsiella pneumoniae.
Genomic sequencing showed that, over about two years, a subpopulation of the bacteria acquired point mutations that thickened its capsule, allowing it to evade the immune system and spread from the urinary tract to the brain. The researchers coined the term “heterovirulence” (or “heterovirulencia”) to describe the coexistence of bacterial subpopulations with markedly different virulence within a single patient.
The findings were reported by scientists from the University of Cologne, the German Center for Infection Research (DZIF) and the Broad Institute of MIT and Harvard in the New England Journal of Medicine. Experiments in mouse models confirmed that the hyper‑virulent capsule‑enhanced strain was lethal, while the typical strain was not. The case highlights how chronic urinary infections can evolve into invasive, highly aggressive disease.
