Kymenlaakso to implement personalized doctor and nurse model
The Kymenlaakso welfare area is implementing a new primary healthcare model where every resident is assigned a dedicated doctor and a nurse working as a team. The rollout will begin in September at the Hamina and Kotka Peninsula health centers.
Under this model, the doctor is responsible for medical treatment, such as examinations and prescriptions, while the nurse manages nursing care, including symptom assessment and chronic disease monitoring. The goal is to improve continuity of care by ensuring patients frequently interact with the same professionals who are familiar with their medical history.
The assignment of medical teams is based on the resident's home location, surname, and random selection. Residents will be notified of their assigned professionals during their next contact with the health center. Patients have the option to change their assigned doctor once per year via an online or paper form.
