An investigation in Argentina has reconstructed the technical failure that led to a major health tragedy involving contaminated fentanyl. Prosecutors allege that a three-millimeter hose disconnection in a production room at Laboratorios Ramallo, which manufactured exclusively for HLB Pharma, allowed pathogens to enter the sterile circuit. This contamination resulted in 90 deaths and 44 other victims.

Evidence from seized mobile devices suggests that staff and supervisors were aware of the technical anomaly and chemical irregularities but chose to continue commercializing the ampoules instead of following safety protocols to discard the batch. Rather than following Good Manufacturing Practices, the companies allegedly attempted to mask the issue through thermal processes.

Furthermore, investigators have uncovered a suspected 'cleaning operation' intended to destroy evidence after the outbreak was detected. This allegedly included burning documents, deleting digital chats and servers, destroying substances, and staging a fake robbery at a plant in San Isidro. The investigation also extends to former authorities at ANMAT and INAME, who are accused of failing to act on multiple warnings regarding the plant's operational irregularities.
