Thirlwall Inquiry finds complete failure to protect babies at Countess of Chester Hospital
The Thirlwall Inquiry, led by Lady Justice Kathryn Thirlwall, has released its final report regarding the crimes of former neonatal nurse Lucy Letby at the Countess of Chester Hospital. The inquiry concluded there was a ‘complete failure to protect babies’ on the neonatal unit due to dysfunctional management and governance.
Findings indicate that some infants could have been saved if hospital staff and management had acted sooner when children began collapsing or showing signs of harm. The report highlighted a failure to invoke safeguarding procedures even when staff were suspected of deliberate harm. Specifically, the inquiry noted that if medical professionals had acted on insulin test results during an attempted murder in August 2015, police contact would have been unavoidable.
The judge recommended 17 reforms, including the installation of webcams or video monitors on all neonatal incubators and 24-hour CCTV coverage for insulin storage. The report also criticized the hospital for keeping parents uninformed about mounting suspicions and for mishandling medical files. While the inquiry focused on institutional failures rather than Letby's personal guilt, it noted that removing her victims from the mortality data would have made the hospital's 2015 and 2016 rates consistent with previous years.
