Malaysia hospitals face financial risks under proposed payment shift

Malaysian hospitals may face increased financial risk due to a proposed transition from a fee-for-service model to a fixed payment system for medical treatments. Under the current fee-for-service model, providers face minimal risk as they are reimbursed for the volume of care provided, which some experts suggest can incentivize unnecessary tests and procedures.

The proposed fixed payment system, which could involve a diagnosis-related group payment model, would require hospitals to cover all costs within an agreed price for specific procedures or conditions. While this shift aims to control costs, industry leaders emphasize the need to ensure that cost-cutting measures do not compromise the quality of patient care.

Stefan Larsson of the International Consortium for Health Outcomes Measurement noted that fixed payments could grant hospitals more clinical freedom to eliminate ineffective treatments. He cited Sweden as an example, where bundled payments led to a 20% reduction in orthopedic surgery costs while allowing hospitals to remain profitable.
