Mario Zappia discusses challenges in treating normal pressure hydrocephalus

At the 56th Congress of the Italian Society of Neurology (Sin) in Florence, Mario Zappia, president of Sin, highlighted the ongoing complexities in managing normal pressure hydrocephalus (NPH). Despite being recognized for over sixty years, the pathology lacks clearly defined parameters regarding which patients benefit most from neurosurgical shunt procedures.

Zappia emphasized the need for closer collaboration between neurologists, who diagnose and quantify the condition, and neurosurgeons, who perform the surgical treatments. He noted that clinical and instrumental investigations, such as CT scans or MRIs, are essential to distinguish NPH from neurodegenerative diseases like Parkinson’s.

Key diagnostic challenges include the fact that over 70% of NPH patients exhibit parkinsonism, which can lead to misdiagnosis. The condition is typically characterized by a ‘classic triad’ of cognitive, gait, and urinary disturbances, alongside ventricular dilation visible through imaging.
