Medicaid to implement new community engagement requirements in 2027
New federal Medicaid community engagement requirements, established by the H.R. 1 legislation passed in 2025, are set to take effect on January 1, 2027. Under these rules, most Medicaid enrollees will be required to document at least 80 hours of qualifying activities, such as work, volunteering, or attending school.
Individuals with serious medical conditions or disabilities may qualify for a medical frailty exemption. However, the law specifies that having a complex medical condition alone may not suffice; the condition must be proven severe enough to impede the person’s ability to participate in the required activities.
Advocates and researchers note that states are already facing administrative challenges in managing disability coverage. Current cases involving the loss of Medicaid disability coverage due to administrative rulings highlight potential difficulties for enrollees as states prepare to implement and review these new federal exemption requirements.
