Medicaid work requirements trigger coverage concerns in Nebraska and Illinois

States across the United States are preparing to implement new Medicaid work requirements, leading to concerns regarding coverage loss and administrative confusion. In Nebraska, which began enforcing these rules early, approximately 70,000 adults covered through Medicaid expansion face requirements to log 80 hours of work, school, or volunteering monthly, or earn at least $580. While Nebraska officials have conducted extensive outreach via mail and text, providers report anxiety among enrollees.

In Illinois, officials are preparing for requirements to take effect in roughly 100 days under the One Big Beautiful Bill Act. Cook County Board President Toni Preckwinkle warned that these changes could leave millions uninsured and shift financial burdens from the federal government to states, local governments, and hospitals. Preckwinkle noted that losing coverage often leads residents to delay medical care, potentially worsening health conditions and increasing long-term costs.
