Medical institutions face financial strain in Japan and US

In Japan, a survey by the National Federation of Health Insurance Medical Practitioners indicates that over 60% of medical institutions have seen a decrease in profits. Despite a 3% increase in medical fee revisions, small-scale clinics are struggling with rising costs and wage demands. Approximately 70% of clinics reported decreased profits as of August, and clinic bankruptcies are reportedly reaching record levels.

In the United States, a study by the Pioneer Institute suggests that hospitals participating in the 340B Drug Pricing Program are spending a lower percentage of their operating budgets on charity care compared to non-participating hospitals. While the program was designed to support rural and safety-net providers, research indicates that sales under the program grew by 152% between 2020 and 2025, generating significant profits for large healthcare systems.
